Our mission is to enable a step change in the prevention of heart attacks and strokes, by making it much easier for clinicians to spot patients who are at high risk of CVD, and to make sure they get on - and stay on - the right treatment.

CVDACTION - the smart data tool, with embedded resources and specialist support for local implementation - has been designed to help local health systems deliver this mission.

Demonstrator and accelerator sites​

During 2024, with funding from NHS England (London), we deployed CVDACTION in 10 Primary Care Networks (600,000 population). The demonstrator programme included the smart data tool together with structured support for workforce and pathway transformation.

We found that CVDACTION:​

• Is accepted well by primary care teams,

• Stimulates substantial change in ways of working,

• Enables a focus on targeting health inequalities,

• Increases staff satisfaction,

• Shows early evidence of step change improvement in treatment optimisation.

Health economic modelling​

We have commissioned an independent analysis to answer the question: if CVDACTION increases uptake of high impact preventive treatments in hypertension, CVD, diabetes and chronic kidney disease:​

• How many heart attacks, strokes and other cardiovascular events will be prevented?

• What will be the costs and savings in health and social care?

• What will be the impact on economic activity and the wider UK economy?
