CVDACTION is a smart risk stratification tool that makes it easier for clinicians to spot patients who are at high risk of CVD and make sure they get the right treatment.

This will drive a step change in the prevention of heart attacks and strokes, and help deliver the UK Mission to reduce deaths from CVD by a quarter in ten years.

 

CVDACTION:

  • Delivers highly actionable data, presented simply, with integrated clinical resources.

  • Built for EMIS, SystmOne and SQL, it can be deployed at practice, network or system level, wherever primary care data is hosted.

  • Complements CVDPREVENT, the national audit, with focus on high risk conditions and high impact treatments that prevent heart attack and stroke

  • Takes comprehensive GP data and integrates into simple dashboards 

  • Each dashboard identifies patients on suboptimal treatment or not treated to target

    • Risk stratifies by clinical priority and risk, translating large numbers of patients into manageable bites of work

    • Makes it easy to prioritise what’s urgent and what can be safely phased over time

    • Makes it easy to optimise multiple conditions/risk factors at the same time

    • Enables holistic care for the patient and efficient use of capacity for clinicians

  • Data is filtered by deprivation, ethnicity, severe mental illness, gender and age so that health inequalities can be targeted

  • Helps GP teams achieve QOF targets and meet the PCN contract requirement to use intelligence derived from risk stratification tools to improve care and outcomes
